Rationale: Patients with acquired brain injury (ABI) may suffer from persistent cognitive deficits and/or subjective cognitive complaints, especially in the domains of attention and working memory. Cognitive deficits are associated with anxiety and depression and may affect social participation and health-related quality of life (HR-QoL). Approximately 25% of the patients with ABI will be referred to an in- and/or outpatient rehabilitation center for multidisciplinary therapy to optimize recovery. Multiple studies suggest that supervised computerized cognitive training (CCT) may enhance cognitive functioning in patients with ABI. Recently, the CCT program RehaCom was introduced as an online version which is suitable for home training. In this study the feasibility and outcomes of implementing home-based CCT into a blended care pathway will be investigated in patients receiving outpatient rehabilitation therapy after ABI. Objective: The aim of this study is to assess the feasibility and to evaluate the effect of blending a home-based CCT program (RehaCom) in standard care on cognitive functioning in patients after ABI. The secondary aim is to evaluate the effect of this CCT program on subjective cognitive complaints, self-efficacy, psychological outcome measures and HR-QoL. Study design: Randomized cross-over trial comparing a 5-week blended care pathway to 5 weeks of standard care within 30 patients with ABI. Study population: Adults with ABI receiving outpatient rehabilitation therapy. Intervention: A blended care pathway including 1 cognitive strategy training session of 1 hour per week in the outpatient rehabilitation center in combination with home-based CCT in 4 sessions of 30 minutes per week, during 5 weeks. The standard care pathway includes 2 cognitive training sessions of 1 hour per week in the outpatient rehabilitation center during 5 weeks. Main study parameters/endpoints: Cognitive functioning (attention and working memory), self-efficacy, psychological functioning (coping, anxiety, depression) and HR-QoL, using non-invasive neuropsychological tests and standardized online questionnaires. All outcomes will be assessed at baseline (T0), after 6 weeks (T1) and after 12 weeks (T2). Nature and extent of the burden and risks associated with participation, benefit and group relatedness: In the blended care pathway patients will be instructed to follow a home-based CCT program. Training at home requires a time investment from patients but will also reduce the number of visits to the rehabilitation center. Participants can choose what time of the day is most convenient for them to engage in the program in their home environment instead of traveling to the rehabilitation center for scheduled cognitive training. Increasing patients' responsibility in their recovery process may improve their self-efficacy and HRQoL. Completion of online questionnaires also requires a certain time investment from patients and might lead to temporary fatigue. Patients may take a break at any moment and continue completing the questionnaires at a later time. By a maximum duration of 45 to 60 minutes per measurement we aim to minimize the burden for patients. There are no risks associated with participation.

This study aims to evaluate the effectiveness of soft tissue augmentation using connective tissue graft (CTG) in pontic site defects. It compares clinical and volumetric outcomes between sites with an existing fixed partial denture (FDP) and sites without an FDP. Following tooth extraction, alveolar ridge resorption often leads to soft tissue defects that can compromise esthetics, function, and prosthetic outcomes. CTG is considered the gold standard for soft tissue augmentation due to its predictable improvement in tissue thickness and stability. In this non-randomized clinical trial, patients with single edentulous sites in the esthetic zone will undergo CTG using a standardized surgical technique. The primary outcome is keratinized tissue thickness, while secondary outcomes include keratinized tissue width, volumetric soft tissue changes, esthetic evaluation (Pink Esthetic Score), plaque index, post-operative pain, and patient satisfaction. Clinical and digital assessments will be conducted preoperatively and at follow-up intervals up to 6 months. The study aims to determine whether performing CTG around an existing bridge provides comparable or improved outcomes compared to sites without a bridge, potentially offering a less invasive alternative to prosthetic replacement.

Although breast cancer remains one of the most common and fatal cancer types among women worldwide, earlier diagnosis and improved therapeutic approaches have led to decreased mortality over last years. Indeed, the use of HER2-targeted drugs has improved clinical outcomes for patients with HER2 positive breast cancer, while, on the other hand, until recently, little progress has been made for HER2-negative (IHC score 0 and 1+) patients. However, it seems that among these HER2-negative breast cancers, substantial heterogeneity exists regarding the expression of hormone receptors (HR) and HER2. Biomarkers are critical for translating the biological heterogeneity of breast cancer into prognostically and therapeutically useful information. In recent years gene expression profiling and genomic analysis have shown utility in specific clinical scenarios, but immunohistochemistry (IHC) remains the cornerstone of biomarker testing in both early and advanced/metastatic disease. In breast cancer, achieving efficacy with endocrine therapy or with HER2-blockade requires identifying patients whose tumors show significant survival dependency on the therapeutic target. This is achieved by using appropriate biomarkers cut-offs ensuring a favorable benefit-to-toxicity ratio for specific patients. This paradigm has recently been challenged by a new class of HER2 drugs that use target expression not as a direct molecular lever but as a vehicle to deliver potent agents to cancer cells, breaking the straightforward link between the molecular target and the corresponding therapy. These drugs exhibit impressive activity at marker expression levels much lower than those required to effectively block HER2 signaling, shifting the diagnostic focus to the lower end of the staining spectrum, specifically distinguishing between HER2-zero and HER2-low expression. Given the lack of a definitive molecular hallmark for cancers characterized by low HER2 expression, ongoing efforts aim to understand the biology of this heterogeneous group of tumors. This understanding is crucial to triage treatment, investigate resistance mechanisms, and inform potential combination strategies. To enrich the available data, we perform a retrospective analysis of HER2-low patients in a large prospective cohort of early breast cancer patients from Greece enrolled in 7 randomized and observational clinical studies. The study is structured as follows: From each patient with early breast cancer, at least two 2.5mm wide cores are punched from donated FFPE blocks and transferred onto low-throughput TMAs. The cores are selected to enrich for regions with higher tumor nucleated cellularity to facilitate NGS analyses. Each core is assessed for biomarker expression with biomarker cutoffs applied at the core level. A case is considered positive for a biomarker if any of its cores tested positive at the predefined cut-off value. Cases showing any core with HER2 IHC 3+ and/or FISH amplification are excluded from this analysis. The remaining cases (n= 2751) are categorized into HER2-low and HER2-zero. The study aims to investigate whether HER2-low and HER2-zero cases exhibit distinct characteristics, suggesting HER2-low may represent a unique biological subtype. Given the importance of the hormone receptors and the known differential distribution of ER/PgR status between these two HER2 categories, after analyzing the entire population cohort, ER/PgR status will be treated as a confounding variable in the analysis (Luminal vs. TNBC) and the HER2 categories will be compared separately within ER/PR-pos (Luminal) and ER/PR-neg (TNBC) disease. To find out whether intra-tumor discordance of HER-2 is suggestive of true biological heterogeneity we will also consider for the analysis the different HER2 IHC scores between the available cores for each case as well as the distribution of ERBB2 CN data (median of ERBB2 avg copies).

The goal of this feasibility trial is to learn if exoskeleton or robotic walking works to reduce nerve (neuropathic) pain after spinal cord injury. This study asks is: * Providing walking practice through use of a robotic device (exoskeleton) three times per week for twelve weeks possible to deliver? * Would people sign up and stick to the programme? * And will it help to reduce neuropathic pain levels after spinal injury? Researchers will compare robotic walking and a relaxation program to see if robotic walking works to reduce neuropathic pain levels after spinal injury. Participants will: * Complete a number of questionnaires and tests related to their pain before the trial. * Complete robotic walking or a relaxation program three times per week for twelve weeks. * Complete the same questionnaires and tests after the trial finishes and 6 months after. * Complete an interview telling researchers about their experiences of the trial.

The aim of this study is to analyze the effects of isometric handgrip training (IHT) on blood pressure of hypertensive patients with peripheral arterial disease. Will be recruited 30 patients which will be randomized into 2 groups (15 per group), IHT and control group (CG). IHT group will training isometric contraction for 12 weeks. In CG patients will be encouraged to increase the level of physical activity. In the pre and post-intervention cardiovascular measures will be obtained.

This study will utilize ctDNA methylation detection to evaluate patients with high-risk T3N0 stage II colorectal cancer who are ctDNA-positive one month after surgery. It aims to investigate the impact of different adjuvant chemotherapy regimens on ctDNA clearance rates and their prognostic significance. By using postoperative ctDNA status to identify patients at high risk of recurrence, the study seeks to implement intensified chemotherapy strategies (treatment escalation) at an early stage, thereby improving ctDNA clearance and ultimately enhancing patient outcomes

This study focuses on understanding how well patients who have recovered from an ICU stay absorb nutrients when receiving enteral (tube) feeding. Proper nutrition is crucial for recovery, but we don't fully understand how efficiently enteral feeding works in ICU survivors. The study will use advanced techniques like bomb calorimetry to measure the energy content of stool, and indirect calorimetry to measure patients' resting energy expenditure (REE). This will help assess the effectiveness of enteral feeding in these patients, providing valuable information about their metabolic needs and nutritional status. The study will also look into the environmental impact of enteral feeding, particularly food waste. By understanding how much of the nutrition is absorbed versus excreted, the study hopes to suggest more sustainable feeding practices and reduce unnecessary waste in hospitals. Key Goals: * Primary Goal: Measure how much energy from enteral feeding is absorbed by patients post-ICU by analyzing their stool and energy expenditure. * Secondary Goal: Assess how enteral feeding can be made more sustainable, with less waste generated from unused nutritional products. This observational study aims to evaluate skin conductance monitoring as a continuous method for pain assessment in postoperative neonates. Pain assessment in newborns is challenging due to their inability to communicate, and current methods rely on intermittent observational scales such as the Neonatal Pain, Agitation and Sedation Scale (N-PASS) in combination with physiological parameters. Skin conductance reflects sympathetic nervous system activity and provides a continuous, objective measure of stress and pain. This study will investigate the correlation between skin conductance measurements and standard clinical pain assessment tools (N-PASS and vital parameters), as well as explore the potential analgesic effect of skin-to-skin care. The study is conducted in a neonatal intensive care unit (NICU) setting where all monitoring and treatments are part of routine clinical care.

The purpose of this study is to assess the effectiveness and safety of a single dose of IPN10200 compared to placebo (double-blind phase) and how well and safely repeat doses of IPN10200 work over time (open-label phase) in adult participants with moderate to severe glabellar lines. Glabellar lines are wrinkle-like lines that appear between the eyebrows and can become more noticeable with age or repeated facial expressions. They may affect a person's appearance and confidence. All participants in the double-blind phase will receive IPN10200 or placebo during the first treatment cycle. De novo participants in the open-label phase will receive IPN10200 during the first treatment cycle. Some participants may receive additional treatment cycles with IPN10200 depending on their eligibility. There will be 3 periods in this study: * A screening period (up to 20 days) to assess whether the participant can take part, requiring at least 1 visit to the study centre. * A treatment period where participants may receive up to 4 treatment cycles. In the double-blind phase, participants receive a single treatment of IPN10200 or placebo. In the open-label phase (rollover participants from double-blind), eligible participants may receive additional cycles of IPN10200. In the open-label phase (de novo participants), participants will receive IPN10200 in the first cycle and eligible participants may receive additional cycles of IPN10200. Requires multiple visits during the first month followed by 1 visit every month. * A follow-up period (24 weeks) after the last injection where participants' health will be monitored. Participants will undergo health measurements and observation, including blood sampling, physical examinations, clinical evaluations and electrocardiograms (ECG: recording of the electrical activity of heart). They will also be asked to fill in questionnaires and keep a diary. Each participant will be in this study for up to 107 weeks. Participants may withdraw consent to participate at any time.

The purpose of this study is to examine how breathing air with different oxygen concentrations (higher or lower than normal) during high-intensity interval training affects: * Aerobic capacity (VO₂max) * Ventilatory thresholds * Blood lactate levels * Perceived exertion This research aims to better understand how oxygen availability influences physiological adaptations to exercise. If you agree to participate, you will undergo the following: Baseline testing: * Cardiopulmonary exercise test (cycling until exhaustion) * Measurement of oxygen consumption, heart rate, and ventilatory thresholds * Blood lactate measurement (finger prick) Training intervention (3 weeks): * 3 sessions per week (total of 9 sessions) * High-intensity interval training (Tabata protocol: 8 × 20 seconds effort / 10 seconds rest) * Exercise performed on an air-resistance cycle ergometer During training, you will breathe either: * Hyperoxic air (high oxygen concentration) or * Hypoxic air (low oxygen concentration) You will be randomly assigned to one of these conditions.
