Towards Targeting the ORigin of the Inflammatory Cascade in Allergic Asthma

NCT04264377

Allergic asthma is a complex and heterogeneous disease caused by excessive responses to inhaled allergens. Current medication, including corticosteroids and bronchodilators, does not act on the origin of inflammation but rather combats symptoms, leaving many patients uncontrolled. Airway epithelium is critical for the initiation and progression of asthma pathology. We will include a 52 subjects divided over two groups: ongoing asthma (26 patients) and non-asthmatic healthy controls (26 subjects) in a cross-sectional study. All subjects will be extensively clinically characterized including respiratory symptoms/questionnaires, in- and expiratory CT-scans, and parameters of large and small airway function and inflammation. In addition, blood and nasal epithelial brushes will be obtained to study the genetic and epigenetic mechanisms of asthma. Finally, bronchoscopy with bronchial biopsies and brushes will be performed under conscious sedation. Bronchial biopsies from both patient groups will be used for single cell transcriptional analysis.

Benign Joint Hypermobility Syndrome is a hereditary connective tissue problem characterized by the emergence of multiple musculoskeletal problems in hypermobile individuals without a systemic rheumatologic disease. Studies have shown that hypermobility may also affect lung function. Playing wind instruments requires tiring breathing activity. The effect of wind instrument on lung function and how it affects lung function is a highly controversial issue. Given the distribution and importance of collagen tissue in the body, multiple effects of hypermobility are inevitable. The aim of this study was to evaluate the presence of hypermobility in young healthy individuals; The aim of the study is to investigate the effects of hypermobility on the cardiovascular system and to provide training to the participants regarding the findings.The aim of this study is to investigate the relationship of hypermobility with respiratory functions in professional hypermobile musicians and to compare this relationship with healthy hypermobile individuals who are not engaged in music.

Cost Effectiveness of Outpatient Set-up of Automated NIV in Obese Patients With Chronic Respiratory Failure

NCT02342899

Obesity is an escalating issue, with an accompanying increase in referrals of patients with obesity-related respiratory failure. Currently, these patients are electively admitted to hospital for initiation of non-invasive ventilation (NIV), but it is unknown whether outpatient initiation is as effective as inpatient set-up. The investigators hypothesise that outpatient set up using an auto-titrating NIV device will be more cost effective than nurse-led inpatient titration and set-up. The investigators will undertake a multi-national, multi-centre randomised controlled trial. Subjects will be randomised to receiving usual inpatient set-up, which will include nurse-led initiation of NIV or outpatient set-up with an automated NIV device. Subjects will be stratified according to trial site, gender and previous use of NIV or continuous positive airway pressure. Assuming 10% drop out rate, a total sample of 82 patients will be required. Cost effectiveness will be evaluated using standard treatment costs and health service utilisation and using health related quality of life measures (SRI and EQ5D). Change in the severe respiratory insufficiency (SRI) questionnaire will be based on analysis of covariance (ANCOVA) adjusting for the baseline measurements between the two arms of patients.

Analysis of Peripheral Blood ILC2s and Th2 Cells in Response to ANB020

NCT04256044

This exploratory laboratory only study will assess alterations in immune cells in blood samples that may be responsive to an inflammatory mediator in asthma called IL-33 (Innate lymphoid type 2 cells, basophils and Th2 cells) in response to treatment with ANB020/placebo (a drug that targets IL-33, in the linked clinical trial ANB020-004). The study will involve additional 10ml blood draws on scheduled study visits in the ANB020-004 clinical trial at baseline, 1 week, 5 weeks, 18 weeks (4 visits total). Total number of subjects is 24 at the same 3 participating UK sites as the main linked clinical trial ANB020-004. Blood samples will be sent on the same day to a flow cytometry laboratory at the NIHR Leicester Biomedical Research Centre (BRC) - Respiratory. A dedicated post doctoral, trained laboratory scientist will analyse the blood samples for the required cell types using a technique called flow cytometry - this technique identifies cell surface and intercellular markers, that together provide a unique cell identity. The experiments will be supervised by a senior research scientist and flow cytometrist Professor Dave Cousins, at the University of Leicester and results of the experiments will be fed back to AnaptysBio (the linked clinical trial Sponsor). Professor Salman Siddiqui at the University of Leicester will oversee the clinical conduct of this laboratory study and act as the Chief Investigator. Professor Siddiqui is also the local site lead Principal Investigator at Leicester for the linked ANB020-004 clinical trial described below. The linked study (ANB020-004) is a Double-blind Placebo-Controlled Proof of Concept Study to Investigate ANB020 (a drug that targets an inflammatory mediator called IL-33) Activity in Adult Patients with Severe Eosinophilic Asthma. The expected duration of the study is up to 141 days. Screening period of 7-14 days and treatment and follow-up period of 127 days.

Vital Sign Monitoring With Continuous Pulse Oximetry and Wireless Clinician Notification After Surgery

Patients are at risk of respiratory depression after having surgery. The medications that patients are treated with to control their pain can impair their breathing and this can progress to respiratory and cardiac arrest and even death. Vital signs assessment on surgical wards is usually done every 4 hours and this may be insufficient to identify and manage many cases of respiratory depression. The aim of this study is to determine the impact on safety and nursing workflow of a respiratory monitoring on two surgical wards by measuring safety outcomes. Respiratory depression is a serious complication of pain treatment that can lead to patient complications and death. The level of monitoring available in hospitals by nursing staff is insufficient to manage this problem. If this new monitoring technology works as designed then patient safety can be improved while maintaining effective pain therapy.

Vitamin D to Improve Outcomes by Leveraging Early Treatment

Vitamin D deficiency is a common, potentially reversible contributor to morbidity and mortality among critically ill patients. We conducted a randomized, double-blind, placebo-controlled, phase 3 trial of early vitamin D3 supplementation in critically ill, vitamin D-deficient patients who were at high risk for death. Patients screened as vitamin D deficient (\<20 ng/mL) were randomized. Randomization occurred within 12 hours after the decision to admit the patient to an intensive care unit. Eligible patients received a single enteral dose of 540,000 IU of vitamin D3 or matched placebo. The primary end point was 90-day all-cause, all-location mortality.

Smartphone Enabled Detection of Nocturnal Cough Rate and Sleep Quality as a Prognostic Marker for Asthma Control

The purpose of the study is to explore the value which cough rate might provide for asthma self-management. In this study, the focus will be specifically on nocturnal cough rate. The plan is to use a longitudinal study design, in order to investigate to which extent trends in the nocturnal cough rates might have meaningful implications for future asthma control and asthma exacerbations of patients. The incidence of nocturnal cough in asthmatics will be described and visualized over the course of one month in the first stage of the study. Additionally, the aim will be to identify and model trends in nocturnal cough rates. Measuring cough is very time-consuming. Currently, there are no cough frequency monitors available, which measure cough rates in a fully automated and unobtrusive way. Consequently, manual labeling of cough based on video or sound recordings is still considered to be the gold standard for measuring cough rates by medical guidelines. Recently, a machine learning algorithm was successfully designed to automatically detect cough in a proof of concept study. This machine learning algorithm will be further developed in order to provide robust results in the field. The focus of this study will be the cough during the night time due to the limited interfering noise, which greatly facilitates manual labeling and enables a more reliable detection rate of the machine learning algorithm. Apart from developing a machine learning algorithm for cough detection, data will be gathered for the assessment of patient's sleep quality based on data obtained from smartphone's sensors.
