PRIMARY OBJECTIVE: I. Assess efficacy of treatment with enfortumab vedotin (EV) in radioactive iodine refractory (RAIR) differentiated thyroid cancer (DTC), anaplastic thyroid cancer (ATC), medullary thyroid cancer (MTC) and Nuclear Protein in Testis (NUT) carcinoma (NC) patients by evaluating the proportion of patients alive and without progression at 6 months. SECONDARY OBJECTIVES: I. Assess efficacy of treatment with enfortumab vedotin (EV) in RAIR DTC, ATC, MTC and NC patients by evaluating the objective response rate (ORR) by imaging. II. Assess efficacy of treatment with enfortumab vedotin (EV) in RAIR DTC, ATC, MTC and NC patients by evaluating progression free survival. III. Assess efficacy of treatment with enfortumab vedotin (EV) in RAIR DTC, ATC, MTC and NC patients by evaluating the overall survival time at 6 months. IV. Assess efficacy of treatment with enfortumab vedotin (EV) in RAIR DTC, ATC, MTC and NC patients by evaluating the overall survival time. V. Assess the safety and tolerability of EV monotherapy in patients with RAIR DTC, ATC, MTC and NC. VI. Characterize the quality of life in RAIR DTC , ATC, MTC and NC patients treated with EV. EXPLORATORY OBJECTIVES: I. Assess immune cell landscape in EV treated patients. II. Assess expression of Nectin-4 in patients with RAIR DTC, ATC, MTC and NC. III. Next generation sequencing (NGS), biomarker analysis and optional single cell sequencing for patients with RAIR DTC, ATC, MTC and NC for assessment of response or failure to EV monotherapy. OUTLINE: Patients receive enfortumab vedotin intravenously (IV) on days 1, 8, and 15 of each cycle. Cycles repeat every 28 days for up to 2 years in the absence of disease progression or unacceptable toxicity. Patients undergo computed tomography (CT) scan, x-ray imaging or magnetic resonance imaging (MRI) and blood and urine sample collection throughout the study and optionally undergo biopsy at progression. After completion of study treatment, patients are followed up at 30 days and every 3 months for 12 months.
