The pathophysiology of SLE (as B-cell activity and autoantibody production are central to the pathogenesis of the disease), along with the plasma cell depleting and proven immunosuppressive activity of Belantamab mafodotin in patients receiving SARS-CoV-2 vaccination suggest that belantamab mafodotin could be a potent agent for patients with SLE. Thus, there is a strong rationale to support the investigation of the activity of belantamab mafodotin in combination with SoC background therapy in patients with previously treated/ refractory SLE, as it could provide a valid option for patients who urgently need new treatments with efficacy beyond what the SoC currently provides. Rationale for belantamab mafodotin dosage and schedule Single agent belantamab mafodotin had initially demonstrated a strong activity with a well-defined manageable safety profile in heavily pre-treated participants with RRMM (2.5 mg/kg Q3W schedule via IV administration). On the other hand, a standard dose for belantamab mafodotin in previously treated/refractory SLE has not been determined yet, therefore the treatment protocols from the DREAMM-9 phase I dose and scheduled evaluation trial of belantamab mafodotin and the results from more recent trials of Belantamab mafodotin are taken into account. This is not an event driven trial. The end of the trial will be at 2 years (24 months) after the last participant enrolled receives the first trial treatment, or after the last treatment dose administered within the context of the trial, to any enrolled participant, whichever occurs first. The trial is estimated to end in December 2030. Planned Duration of trial intervention is 36 months. Participants will be treated for a total period of two years per participant or until treatment failure, physician decision, unacceptable toxicity, withdrawal of consent, or death (whichever occurs first). NOTE 1: The start of a Cycle1 D1 of a cycle is defined when Belantamab mafodotin is administered.
